Survey Instruments and Codebooks Registration/Login

SIAA parent and adolescent survey instruments are available for viewing in PDF files. Questionnaires are available in English and Spanish language. In addition, codebooks for the instruments will be also available. The codebooks will contain a description of each variable.

For reference, index files list all the questions and corresponding variable names for the SIAA survey instruments. The index files will also be available in a PDF file. All files that are currently ready are available for downloading after you have completed the on-line registration.
