Terminate Restricted-Use Data Contract

To terminate a Restricted-Use Data Contract, the following steps are required:

  • Download and complete the contract termination form and email to .
  • Destroy all data files at the originally-approved site.
  • Return all CDs containing data files, within thirty (30) days of contract expiration by UPS or FedEx with tracking and signature required.
  • Submit a final annual report or a statement that you have no publications, presentations, dissertations, or grants to report since the last report


Add Health will visit the site within a year of contract termination to confirm the data have been destroyed.
