Terminate Restricted-Use Data Contract

To terminate a Restricted-Use Data Contract, the following steps are required:

  • Notify  of your intent to terminate your contract.
  • Destroy all data files at the originally-approved site.
  • Email  stating that all Add Health data files have been erased with a secure erasure program listed in your security plan.
  • Return all CDs containing data files, within thirty (30) days of contract expiration by UPS or FedEx with tracking to:

                          123 West Franklin Street
                          Chapel Hill, NC 27516
  • Submit a final annual report or a statement that you have no publications, presentations, dissertations, or grants to report since the last report.
  • Add Health will visit the site within a year of contract termination to confirm the data have been destroyed.
