Add an Additional Person to a Restricted-Use Data Contract

To add additional researchers, collaborators, officemates, or information technology staff to your Add Health Restricted-Use Contract. Please log in to your CPC Data Portal application to download and complete the forms. If you do not have a CPC Data Portal application yet, please email  for instructions.

Researcher Supplemental Agreement and Security Pledge

Researchers who will have access to the data may be added to an Add Health Restricted-Use Data Contract by signing the Supplemental Agreement and a Security Pledge. 

Collaborator Security Pledge

Collaborators that do not have access to the data but are involved in the project through discussion of the data, analysis or project and/or having access to printed analysis or project materials should sign the Collaborators Security Pledge

Officemate Security Pledge

Officemates that do not have access to the data or project materials but share an office space with a Researcher who has a Restricted-Use Data Contract should sign the Officemate Security Pledge.

Information Technology Staff Security Pledge

Information Technology staff who will have access to the data but will not use the data for analysis should sign the Security Pledge.

Steps to add researchers to a contract

Add Health now processes all request through the CPC Data Portal. (If you have Add Health forms on file, please discard them, as the Portal will always have the most current forms). To add researchers to a contract, go to CPC Data Portal 

  1. Log In (Right upper corner) and go to “Applications tab”
  2. Click the “User List” button
  3. Fill out all the fields (Last name, First Name, role, email (optional), Access Location)
  4. Click “Add” button
  5. Click on contract ID “xxxxxxxxx” to go back to the requirements page
  6. Go to the requirement: Add Health Supplemental Agreement

    * If this requirement had been previously approved, you will see "Click here to add more documents"
    * Download the blank form  
    * Upload the completed form(s)

  7. Go to the requirement: Add Health Supplemental Agreement

    * If this requirement had been previously approved, you will see "Click here to add more documents"
    * Download the blank form  
    * Upload the completed form(s) 

Information about Add Health will be distributed through the Add Health list server.

To subscribe to the listserver:

  1. Send an email to  with no message body
  2. Wait for addhealth confirmation email
  3. Click the confirm link in the email
