As one of us with colleagues (RSC) (Jan 17, p 208)1 noted, the health status of populations in developing countries will never be understood until the void in vital statistics is addressed. Paul Kelly and colleagues (March 21, p 883)2 report data from Lusaka that show mortality among adults was exceptionally high because of AIDS, yet these data raise the question of a reference standard to determine what is high.URL
